"i-CAT® Give-away" Sweepstakes Terms and Conditions: "i-CAT® Give-away" Sweepstakes open from March 1, 2010 until the close of business on March 31, 2011. NO PURCHASE NECESSARY TO ENTER. PURCHASE WILL NOT INCREASE THE ODDS OF WINNING.
One (1) winner will be selected in a random drawing from all properly completed entry forms. Entries may be submitted by 1) completing and mailing a printed business reply card, 2) completing and submitting a printed entry card at the ISI tradeshow booth, or 3) completing an on-line form at www.imagingsciences.com. The drawing will be held on or about May 31, 2011 by the sponsor, Imaging Sciences International, LLC ("ISI" or "Sponsor"), whose decision is final. Participant need not be present to win.
Sweepstakes open to practicing US-resident dentists only. One entry per person, and the sweepstake will not be combined with any other ISI promotions. Odds of winning are determined by the number of entries received. ISI is not responsible for lost, late, illegible, unintelligible, incomplete, undelivered, stolen, postage due or misdirected entries.
Prize is one (1) i-CAT digital imaging device ($169,999 prize value) with a one (1) year warranty. Sponsor shall only be responsible for awarding the number of prizes as stated in these Terms and Conditions. Prize is non-transferable, cannot be substituted and has no cash alternative. The i-CAT® will be shipped and installed by Henry Schein, Inc.
By entering this Sweepstakes, participant acknowledges that the information provided may be used to communicate with the participant in relation to this Sweepstakes as well as for other purposes. Participants DO NOT have to submit to a sales presentation.
Sponsor expressly disclaims all responsibility or liability for loss, damages, injury or delay to property or persons due to any causes, including but not limited to, acts of God, nature, labor strikes, and terrorism occurring during the use of the prize awarded. Sponsor makes no representations or warranties concerning the appearance, safety or performance of any prize awarded.
By entering this Sweepstakes and/or accepting a prize, participants agree to release, discharge, indemnify, and hold harmless Imaging Sciences International, LLC, its affiliates, parents, subsidiaries, advertising, design, and promotion agencies, and their respective officers, employees, representatives, and agents, from any injuries, losses, liability, or damages of any kind resulting in whole or in part from acceptance, possession, or use of any prize, or participation in this Sweepstakes.
Under no circumstances will participants be permitted to obtain awards for, and participants hereby waive all rights to claim, punitive, incidental or consequential damages, attorney's fees or any right to have any damages multiplied or increased.
Winner will be notified in writing at the mailing address or e-mail address listed on the entry form and announced at www.imagingsciences.com. Winner further grants to Sponsor and Sponsor's agents and assigns, without limitation, the right to use Winner's name and address for promotional purposes in any media without further payment or consideration.
All federal, state, local income and/or other applicable taxes, are the winner's sole responsibility. Winner must complete and return an affidavit of eligibility within fifteen (15) days of date on the winner notification letter and comply with all terms and conditions of this Sweepstakes.
